Join HarmonyCares as a Nurse Practitioner (PRN) and deliver high-quality, personalized healthcare in the home setting. You'll conduct comprehensive health assessments, collaborate with patients and providers, and make a meaningful impact in the lives of those with limited access to care.
Key Responsibilities
Conduct comprehensive in-home health risk assessments
Take history, examine, and determine diagnoses
Provide written documentation of patient visits per NCQA standards
Take patient vital signs as necessary
Place case management referrals and communicate with PCP
Communicate with patients, caregivers, and other providers
Perform clinical duties while observing OSHA Universal Precautions
Maintain patient confidentiality
Attend required meetings and participate in committees
Participate in professional development activities
Required Qualifications
Active/unrestricted medical license
Active CPR Certification
Board eligible or board certified in family medicine
Outstanding EHR skills
Preferred Qualifications
Geriatric training/experience
Skill in teamwork and maintaining effective working relationships
