Job Description
Join Monte Nido & Affiliates as a Hybrid Social Worker, where you will play a crucial role in care coordination across multiple treatment programs. This position involves supporting adults and adolescents with eating disorders by facilitating transitions in care, connecting families to resources, and ensuring effective case management.
Key Responsibilities
Partner with the clinical team to offer case management and care coordination for clients and families
Assist in connecting clients and families with relevant community-based programs and resources
Send referrals to community programs and facilitate collaboration with outpatient providers
Help facilitate smooth transitions between various levels of care
Provide necessary information and emotional support to clients and families for transitions
Connect clients with appropriate resources post-discharge
Required Qualifications
Master’s degree in Social Work or related field
Experience with CareOregon/Medicaid
Proven experience in case management or care coordination in behavioral health or residential treatment settings
Strong communication skills
Ability to work collaboratively with a multidisciplinary team
Ability to build relationships with clients, families, and community providers
Understanding of available community resources, government programs, and insurance processes
Preferred Qualifications
LCSW preferred
