Job Description & Requirements

Responsibilities:

• Evaluate and treat patients per physician orders.
• Develop and update treatment and discharge plans.
• Document services and patient progress accurately.
• Supervise COTAs as required.
• Collaborate with care teams and maintain compliance standards.

Qualifications:

• Graduate of an accredited OT program (MOT/OTD) .
• Active Oregon OT license (or eligibility to obtain).
• Strong clinical, documentation, and communication skills.
