Physical Therapist (PT) – Skilled Nursing Facility (SNF)

MedAdventures is seeking a compassionate and motivated Physical Therapist (PT) to join a trusted Skilled Nursing Facility (SNF) team in Bennington, VT . This is a great opportunity for a PT who’s passionate about helping patients regain strength, mobility, and independence in a supportive, team-based care setting.

You’ll work closely with an interdisciplinary team to deliver evidence-based therapy and help residents improve their quality of life through tailored treatment plans.

Key Responsibilities:

• Evaluate residents and develop individualized treatment plans to restore function, reduce pain, and improve mobility
• Deliver skilled physical therapy services in accordance with physician orders and facility protocols
• Monitor progress, reassess goals, and adjust treatment plans as needed
• Educate patients, caregivers, and staff on exercise programs and mobility techniques
• Document all care provided accurately and in a timely manner

Qualifications:

• Active Vermont PT license (or eligibility to obtain)
• Experience in a skilled nursing or long-term care setting preferred
• Strong interpersonal, clinical, and documentation skills
• Ability to collaborate effectively with an interdisciplinary team
• Commitment to patient-centered, outcome-driven care
