Key Responsibilities
Provide intensive home-based therapeutic interventions and support services
Design and implement an Individualized Treatment and Crisis Plan
Conduct suicide assessments and create safety plans
Conduct comprehensive assessments and evaluations
Provide intensive family therapy, psychoeducation and support services
Coordinate families with formal services and natural supports
Complete all required documentation in the Electronic Health Record
Link consumers and families with aftercare providers
Required Qualifications
Master's Degree in Counseling, Social Work, Psychology or related field
Two years paid experience working with children/youth/adolescents
Valid Driver’s License and full-time use of a vehicle
Eligible for Act 33, 34 and FBI clearances
