Job Description

Join UnitedHealthcare as a Field Care Coordinator, where you'll play a vital role in delivering integrated care for individuals in Northwest Wayne County, MI. This position involves developing personalized care plans, advocating for patients, and ensuring efficient service delivery while working remotely and traveling for home visits.

Key Responsibilities

  • Develop and implement care plan interventions as a single point of contact
  • Communicate health-related information to ensure quality coordinated care
  • Advocate for patients and families to ensure their needs are represented
  • Assess, plan, and implement individualized care strategies
  • Identify barriers to care and provide management interventions
  • Initiate referrals for social service programs
  • Provide resource support for local services
  • Manage the person-centered service/support plan
  • Conduct home visits and in-person visits as necessary
  • Maintain compliance with state and federal guidelines

Required Qualifications

  • Current, unrestricted independent licensure as a Registered Nurse (RN) in Michigan
  • Master’s degree and current, unrestricted independent licensure as a Social Worker (e.g., LMSW, LCSW, LLMSW)
  • Bachelor’s degree and current, unrestricted independent licensure as a Social Worker (e.g. LLBSW, LBSW)
  • 2+ years of experience in community health setting
  • 1+ years experience with local behavioral health providers and community support organizations
  • 1+ years experience with long-term care needs and/or home and community-based services
  • 1+ years experience in electronic documentation systems and MS Office
  • Access to reliable transportation for member visits up to 75% of the time
  • Access to quiet workspace at home with ability to secure PHI
  • Ability to travel for quarterly team meetings
  • Must reside within the state of Michigan

Preferred Qualifications

  • RN or LMSW; LCSW, LLMSW
  • 1+ years of medical case management experience
  • Training or certifications in Motivational Interviewing, Stages of Change, Trauma-Informed Care, Person-Centered Care
  • Experience serving individuals with co-occurring disorders
  • Experience with MI Health Link (MMP)
  • Experience in Managed Care
  • Knowledge of NCQA documentation standards
