Job Description

Key Responsibilities
Conduct comprehensive bio-psychosocial assessments and establish care plans
Collaborate with interdisciplinary teams to assess care levels and develop plans
Provide direct patient care including case management and counseling
Monitor progress of interventions and revise care plans as needed
Maintain timely patient-centered documentation
Required Qualifications
Master's Degree in Social Work, Counseling, or Marriage and Family Therapy
LCSW, LPC, LMFT, LADC
Previous experience in a healthcare setting
Valid Driver's License
Computer proficient
Excellent communication skills
Preferred Qualifications
Experience in crisis intervention
Knowledge of health-related social needs and cultural competence
