Job Description
Join The Jewish Board as a Case Manager, where you will support adults with serious mental illness in achieving independence through personalized recovery goals. This role involves advocacy, crisis management, and collaboration with various services while working within a compassionate and culturally competent team.
Key Responsibilities
Carries a caseload and practices person-centered service planning
Evaluates and assesses independent living skills
Advocates on behalf of residents with other service providers
Completes regular and timely documentation of all resident interactions
Participates in planning and execution of special events
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
Preferred Qualifications
At least 1-year experience in Human Services
Familiarity with Microsoft Office suite of products
