Key Responsibilities
Conduct initial and periodic in-person assessments of participants
Recommend and coordinate adaptive equipment and therapy services
Develop and implement individualized treatment plans
Deliver OT interventions across various settings
Educate participants and caregivers on equipment use and home safety
Maintain documentation of assessments and treatment
Participate in daily interdisciplinary team meetings
Train staff in safe OT practices and equipment use
Support quality improvement and compliance initiatives
Required Qualifications
Advanced degree in Occupational Therapy from an accredited school
Current license to practice Occupational Therapy in the state
Two years of experience working on an interdisciplinary team in a hospital, nursing home, or community-based setting
Preferred Qualifications
Experience working with a frail or elderly population
