Job Description
Join The Jewish Board as a Case Associate and make a significant impact in the community. You will be responsible for developing action plans for client care, managing cases, and accessing community resources to support clients and their families.
Key Responsibilities
Function as a case planner to supply appropriate interventions for client care
Develop action plans to utilize existing resources and establish new ones
Conduct routine community contacts and home visits
Evaluate clients for individualized treatment programs
Provide case management and individual/group counseling
Perform community outreach and establish linkages with helpful organizations
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual Haitian-Creole
