Key Responsibilities
Establish trusting relationships with patients and families
Coach patients in managing chronic health conditions
Assist patients with understanding care plans
Visit patients in their homes for assessments
Document activities and service plans
Advocate for patients and liaise with community services
Facilitate communication between providers and patients
Required Qualifications
Must be 18 years or older
High school diploma or GED
U.S. Citizen or Permanent Resident
Must not have prior professional or volunteer experience in healthcare/community health work
Ability to pass a national criminal background check
Preferred Qualifications
Familiarity with Microsoft Office Suite (Word, Excel, PowerPoint)
