? Position Title: Registered Nurse – Home Health

Location: Robinson, IL
Duration: 13 Weeks
Shift: Day Shift (8-hour shifts with weekend on-call rotation)
Guaranteed Hours: 40 per week

? Position Summary:

As a Home Health RN, you will provide skilled nursing care to patients in their homes, coordinating treatment plans that promote independence, safety, and overall well-being. This role requires independent clinical decision-making, time management, and a strong commitment to patient-centered care.

? Key Responsibilities:

• Perform initial and ongoing comprehensive assessments of patients, including physical, psychosocial, and cultural needs.
• Develop individualized care plans in collaboration with the interdisciplinary team; revise based on clinical outcomes.
• Educate patients and caregivers on diagnoses, treatment plans, medications, and self-care strategies; assist with discharge planning.
• Administer medications, manage IV lines and central lines per protocol; evaluate pain management effectiveness and modify care as needed.
• Maintain complete, timely, and accurate documentation in compliance with agency policies and regulatory standards.
