Key Responsibilities
Achieve quality healthcare outcomes through individualized discharge plans

Required Qualifications
Graduate of an accredited school of nursing
Current RN license in the State of Nevada
Minimum three years experience in varied clinical settings
Knowledge of social and physical factors affecting functional status at discharge
Knowledge of community resources for post-discharge needs
Computer proficiency in word processing, spreadsheets, and data management
Preferred Qualifications
Two years experience in Utilization Review, Utilization Management or Case Management
