Join CenterWell Home Health as a Home Health Occupational Therapist, where you'll provide personalized rehabilitation care to enhance patients' strength, mobility, and independence in their homes. Collaborate with healthcare teams to create therapeutic plans that significantly improve the quality of life for your patients.
Key Responsibilities
Assess patients' daily living/work-related skills and develop therapeutic retraining programs
Confer with the patient's physician and other health care team members
Provide therapeutic treatment and instruction to patients
Review/expand retraining programs consistent with patient goals
Consult with vocational team members for vocationally oriented plans
Report patient status and progress to the physician and rehabilitation staff
Participate in care coordination and discharge planning
Design community reintegration activities
Recommend/design special adaptive equipment
Train staff and family members to carry out retraining programs
Coordinate/supervise Occupational Therapy Assistant and Home Health Aide performance
Document patients' care observations and interventions
Required Qualifications
Degree from an accredited Occupational Therapy Program
Current and unrestricted OT licensure
Current CPR certification
Good organizational and communication skills
A valid driver’s license, auto insurance, and reliable transportation
Preferred Qualifications
A minimum of six months of occupational therapy experience
Home Health experience a plus
