Job Description
Join MJHS as an Access Center RN Coordinator and play a crucial role in managing referrals for hospice and palliative care, ensuring timely and safe admissions for patients and their families.
Key Responsibilities
Manage high-tech and complex referrals in the Access Center
Monitor the Access referral list
Work with Access Coordinators to ensure timely admissions to the Hospice program
Communicate with Hospice Team disciplines to ensure safe admissions
Verify all written and verbal orders for evaluation prior to admission
Required Qualifications
Graduate from an accredited school of Nursing
NY State RN License
Minimum of two years Hospice experience
PC Skills
Good written and oral communication skills
Good customer service skills
Preferred Qualifications
BSN
National Board Certification in Hospice and Palliative Care
