Now Hiring: Travel RN Coordinator – Palo Alto, CA
Duration: 26 weeks
Schedule: 10 hour Day Shift, expect weekend and holiday rotations

Important: This is Not hands-on patient care – it is telephone and computer work, supporting different clinics based on need
Key Responsibilities:
• Assesses, triages, and provides nursing consultation, performs follow-up and care coordination during face to face encounters or using telecommunications technologies during encounters
• Provide coordination and care for patients requiring assistance and support in accessing and managing their health care need
• Coordinating patient communication, patient education, and preparing patients and/or caregivers for procedures and tests in collaboration with the health care team
• Ensuring continuity of care and coordination, identifying opportunities to improve patient care services; and assisting clinic management to identify and develop operational needs
• Completes written reports and projects within appropriate time frames. Reviews tasks, sets priorities, and revises plans for effective time management
Qualifications:
• Minimum 2+ years of experience in Outpatient Oncology and at least 1 Travel assignment
• Chemo Certification a plus, but not required
• Nurse Navigation experience and Case Mgt experience a plus, but neither required
• Registered Nurse – CA State Licensure Required
• BLS Certification (American Heart Association) required
