Job Description

Join UnitedHealthcare as a Field Care Coordinator to make a significant impact in healthcare by supporting members through personalized care coordination. This role involves developing tailored care plans, advocating for patients, and engaging with community resources while traveling within a 50-mile radius.

Key Responsibilities

  • Develop and implement care plan interventions
  • Communicate health-related information to stakeholders
  • Advocate for patients and families
  • Assess, plan, and implement individualized care strategies
  • Identify barriers to care and provide management interventions
  • Initiate referrals for social services
  • Provide resource support to members
  • Manage person-centered service/support plans
  • Conduct home visits and in-person visits
  • Document member information and care management activities

Required Qualifications

  • Current, unrestricted independent licensure as a Registered Nurse (RN) in Michigan or
  • Master’s degree with current, unrestricted independent licensure as a Social Worker (e.g., LMSW, LCSW, LLMSW) or
  • Bachelor’s degree with current, unrestricted independent licensure as a Social Worker (e.g. LLBSW, LBSW)
  • 2+ years of experience in community health setting
  • 1+ years of experience with local behavioral health providers and community support organizations
  • 1+ years of experience with long-term care needs and/or home and community-based services
  • 1+ years of experience in electronic documentation systems and MS Office
  • Access to reliable transportation
  • Access to a designated quiet workspace for PHI
  • Ability to travel to Southfield, MI for quarterly team meetings
  • Must reside within the state of Michigan

Preferred Qualifications

  • RN or LMSW; LCSW, LLMSW
  • 1+ years of medical case management experience
  • Experience in Motivational Interviewing, Stages of Change, Trauma-Informed Care
  • Experience with co-occurring disorders
  • Experience with MI Health Link (MMP)
  • Experience in Managed Care
  • Knowledge of NCQA documentation standards
