**Job Description:**

Seeking a skilled Home Health Physical Therapist to deliver quality care to patients in their homes. Expertise in HCHB/OASIS documentation is essential.

**Responsibilities:**

• Provide physical therapy to home health patients.
• Document same•day using Homecare Homebase (HCHB).
• Manage 20•25 cases weekly, achieving 5•7 productivity points daily.
• Attend weekly in•person office meetings.
• Be flexible to float as needed.

**Requirements:**

• Valid CA PT License and BLS Certification.
• 2+ years of recent home health PT experience.
• Proficient in HCHB/OASIS, with recent experience.
• Familiarity with Oasis E post•January 2023.
• Available for 40 hours/week, including potential weekends.

**Additional Information:**

• 13•week contract, possible extension.
