Job Description
Join The Jewish Board as a Case Associate, where you'll support clients in building skills for independent living. This role involves case planning, community resource utilization, and direct client support to make a meaningful impact in their lives.
Key Responsibilities
Conduct routine community contacts on behalf of social workers
Locate and utilize appropriate community resources for clients
Evaluate living conditions of clients through home visits
Provide structure and guidance to clients on independent living skills
Run groups under the supervision of the social worker
Evaluate clients for treatment programs including child safety
Facilitate client referrals to treatment or service providers
Perform individual/group counseling based on client need
Identify and provide access to necessary concrete services
Perform community outreach and establish linkages with other organizations
Complete required documentation including progress notes and risk assessments
Required Qualifications
BA Degree
Experience in a community or social service group
Five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual Haitian-Creole
