RN – Case Manager (General Case Management)

Location: Jackson, Michigan 

Contract Duration: 13 Weeks (November 25, 2025 – February 21, 2026) 

Shift: 8-Hour Days, 5 Days per Week 

Guaranteed Hours: 40 per week 

Position Summary We are seeking a skilled RN Case Manager for a 13-week travel assignment in Jackson, Michigan. This role supports physicians and the interdisciplinary team in coordinating patient care with a focus on improving clinical outcomes, patient satisfaction, and cost efficiency. The Case Manager I is responsible for a designated caseload and integrates utilization management, patient education, care facilitation, and discharge planning.

Key Responsibilities

• Facilitate precertification and payor authorization processes
• Coordinate collaborative patient care across the continuum
• Identify and remove barriers to timely and efficient care delivery
• Apply process improvement methodologies to evaluate care outcomes
• Support clinical documentation and serve as a resource for coding accuracy
• Communicate effectively with physicians and care teams
• Demonstrate knowledge appropriate to patient age and development

Required Qualifications

• Active RN license in the State of Michigan
• Bachelor of Science in Nursing (BSN)
• Minimum two years of case management experience (utilization case management not applicable)
• Strong interpersonal, analytical, and time management skills
• Ability to work independently and exercise sound clinical judgment

Preferred Qualifications

• Three years of RN clinical experience
• Experience with Care Manager and Epic systems
• Familiarity with discharge planning, performance improvement, and managed care reimbursement
