Job Title: Physical Therapy Assistant (PTA) – Home Health
Location: Eastland, TX
Duration: 13 weeks(Potential for extension)
Schedule: Monday – Friday, 8:00 AM – 5:00 PM (40 hours/week)

Job Summary:
We are seeking a skilled Physical Therapy Assistant (PTA) to provide home health services in the North-West Valley area. This is a full-time opportunity with no guaranteed hours. Homecare Homebase (HCHB) or online training is required prior to the start date.

Responsibilities:

• Deliver high-quality physical therapy care in patients’ homes
• Document patient care accurately using HCHB (preferred)
• Collaborate with patients, families, and the care team to implement therapy plans
• Maintain all required certifications and licensure

Requirements:

• Current state license to practice Physical Therapy – Required
• Proof of current CPR certification – Required
• Current driver’s license, reliable transportation, and insurance – Required
• 2 years of experience as a Physical Therapy Assistant – Required
• Home Health (HH) experience – Required
• Homecare Homebase (HCHB) training or online course prior to start – Desired

Education:

• Graduate of an accredited school of Physical Therapy Assistant with an Associate Degree in Physical Therapy – Required
