Key Responsibilities
Coordinates post-discharge patient care needs
Completes and documents patient assessments
Develops mutually agreeable plans of care with patients and families
Facilitates resolution of issues for safe transfers
Ensures ongoing contact with interdisciplinary staff
Collaborates with the health care team to meet regulatory standards
Required Qualifications
Registered Nurse License (Oregon)
Registered Nurse License (Washington) or Compact License
Basic Life Support certification
Minimum of two years combined RN experience in Med/Surg, ICU, Emergency Department, Home Health, Skilled Nursing Facilities, Hospice, Long Term Acute Care, Inpatient Rehab, Utilization Management
Preferred Qualifications
Knowledge of appropriate utilization of acute hospital and Kaiser Permanente internal resources
Knowledge of Medicare and Medicaid regulations
Knowledge of utilization management principles and tools
