Now Hiring: Physical Therapist Assistant (PTA) – Skilled Nursing Facility

Location: Mantey Heights – Grand Junction, CO
Schedule: Full-time / Part-time
Reports To: Director of Rehabilitation (DOR) / Supervising Physical Therapist

Position Summary

The Physical Therapist Assistant (PTA) works under the supervision of a licensed Physical Therapist to provide rehabilitative services to residents. The PTA helps implement treatment plans, monitors resident progress, and assists residents in regaining strength, mobility, and independence.

Responsibilities

  • Carry out treatment plans established by the Physical Therapist.
  • Provide therapeutic exercises, functional mobility training, and physical modalities to residents.
  • Document resident progress accurately and in compliance with facility and regulatory standards.
  • Educate residents and families on exercises, mobility techniques, and safe practices.
  • Collaborate with the rehab team, nursing staff, and physicians to achieve resident care goals.
  • Participate in care plan meetings and contribute to quality improvement initiatives.

Qualifications

  • Current Colorado Physical Therapist Assistant license in good standing.
  • Graduate of an accredited PTA program.
  • Experience in skilled nursing or geriatric care preferred, but new graduates are welcome to apply.
  • Strong communication and interpersonal skills.
  • Dedication to teamwork and resident-centered care.
