Travel Long Term Care (LTC) Physical Therapist - $2,220 per week

Key Responsibilities

• Perform patient evaluations, including history, systems review, and selection of appropriate tests and measures.
• Develop and implement individualized treatment plans in collaboration with the interdisciplinary team.
• Conduct ongoing reassessment to ensure optimal outcomes.
• Initiate and manage discharge planning beginning with the first visit.
• Supervise and delegate tasks to assistants and students per state law and facility policy.
• Complete timely and accurate documentation per policy and regulatory standards.
• Provide education to patients, families, and caregivers to support rehabilitation goals.
• Participate in professional growth, continuing education, and quality improvement activities.
• Maintain a safe and therapeutic environment for all patients and staff.
