Key Responsibilities
Deliver skilled physical therapy to patients based on an individualized Plan of Treatment
Evaluate patient needs and develop rehabilitation goals
Adjust treatment plans and techniques as needed
Collaborate with the interdisciplinary home health team
Educate patients and caregivers on exercises and safety
Document all visits, assessments, and progress notes
Support patients in maximizing their independence and improving quality of life
Required Qualifications
Master’s degree in Physical Therapy from an accredited program
Current PT license in the state of employment (or eligibility to obtain)
CPR certification
Strong communication, assessment, and problem-solving skills
Ability to work independently in a variety of home environments
Valid driver’s license, auto insurance, and reliable transportation
Preferred Qualifications
1+ year of experience in a hospital, home health, or outpatient rehab setting
In-home therapy delivery
Geriatric rehab, orthopedic or neurologic rehab
Post-acute care or chronic condition management
Working as part of a mobile or interdisciplinary care team
