Job Description
As a Care Specialist at Upward Health, you'll play a vital role in delivering chronic care management to patients in their homes. Your work will involve engaging with patients, coordinating care, and promoting health to improve their overall wellbeing.
Key Responsibilities
Deliver chronic care management to high-complexity patients.
Engage in direct outreach to patients and guide them through the enrollment process.
Facilitate virtual appointments with providers.
Ensure patients adhere to care plans.
Coordinate care and provide education to improve patient outcomes.
Required Qualifications
Minimum of 2 years of relevant work experience (e.g., Community Health Worker, Peer Support Specialist, Medical Assistant, etc.)
High school diploma or GED required.
A valid driver’s license, auto liability insurance, and reliable transportation.
Experience in chronic care management or working with chronically ill/elderly patients.
Technologically proficient with basic computer skills (typing, using EMR systems).
Strong interpersonal communication skills.
Preferred Qualifications
Experience with motivational interviewing, trauma-informed care, and care coordination.
