Travel Physical Therapist (PT) Location: Arizona (Banner Health)
Shift: 7:00 AM – 7:30 PM (12-hour shift)
Schedule: Guaranteed 24 hours/week
Contract Length: 13 weeks (91 days)
On-Call & Weekend: Schedule TBD by facility
Start Date: Must be on or very close to order start date
Restrictions & Requirements (MANDATORY): - AZ residents (based on driver’s license ) — Not eligible
- Driver's License copy — Must be included in submission
- Experience: Minimum 24 months (2 years) recent PT experience – Required
- License: Active Arizona PT license – Verification must be <30 days old
- Certifications:
- BLS – Must be AHA or American Red Cross only , active upon submission
- Include all certs in submission
- No license hits – Required
- RTO:
- Not allowed during week 1 or on holidays
- Max 7 days total
- All RTO must be listed on Banner cover page
Preferred: - Travel experience
- Open to multiple Banner locations and multiple shifts
- Experience across varied PT departments (check applicable departments on cover sheet)

- All facilities, shifts, states, and departments the traveler is open to
- All RTO listed clearly
- Vaccination status (flu/COVID) noted
- Resume: Detailed, including:
- Facility names
- Facility size, bed count
- Trauma level (if applicable)
- Units worked in
- Skills Checklist: PT-specific, within 1 year
- Certifications & License Verification
- Driver’s License (non-AZ)
Vaccination Policy: - Flu vaccine: Required — Banner declinations available (form must be requested)
- Vaccination status must be noted in submission comments
