About The Position

• Coordinate patient care and discharge planning in an acute hospital setting
• Assess medical necessity per evidence-based guidelines
• Develop and update patient care plans
• Manage disease and needs assessments, including ordering DME
• Ensure compliance with CMS, CPT coding, Department of Health, DRG, and HIPAA regulations

• Minimum 2 years acute medical care management experience in care coordination and discharge planning
• Proficient in Epic charting system
• Travel experience required; first-time travelers considered
