Occupational Therapist (OT) – Home Health (13 Weeks)
Location: Raleigh, NC
Schedule: Monday–Friday | 8:00 AM – 5:00 PM | 40 hours/week
Productivity Expectation: ~30 units/week
On-Call: May be required if requested.

Territory Coverage:
Wake County – primarily Knightdale, Wendell, Zebulon, Raleigh, Wake Forest, and Rolesville.

Position Overview:
We are seeking an experienced Home Health Occupational Therapist capable of managing independent field-based patient visits. The ideal clinician will have strong assessment, care planning, and documentation skills, with the ability to manage a community-based schedule efficiently.

Responsibilities:

• Perform in-home OT assessments and treatment interventions.
• Develop, implement, and adjust individualized care plans to support functional improvement.
• Document clinical observations, interventions, and outcomes accurately.
• Communicate effectively with the interdisciplinary care team to ensure continuity of care.
• Educate patients and caregivers on therapy techniques and safety considerations.

Required Qualifications:

• Current Occupational Therapist license in the state of North Carolina – Required
• CPR/BLS certification – Required
• Valid driver’s license , reliable vehicle, and auto insurance – Required
• Minimum 2 years OT experience in acute care or rehabilitation – Required
• Home Health experience, including OASIS & field-based visits – Required
• Homecare Homebase (HCHB) experience – Preferred
