Join The Jewish Board as a Case Manager to support adults with serious mental illness in achieving independence. You will guide residents in creating personalized recovery goals, advocate for their needs, and collaborate with community services to enhance their quality of life.
Key Responsibilities
Carries a caseload and practices person-centered service planning
Evaluates and assesses independent living skills
Supports residents in achieving medication management independence
Advocates for residents with other service providers
Completes regular documentation of resident interactions
Participates in intake, assessments, and follow-up conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
Preferred Qualifications
At least 1-year experience in Human Services
Familiarity with Microsoft Office suite of products
