As a Care Coordinator at The Jewish Board, you'll play a crucial role in linking individuals with chronic behavioral health and medical conditions to essential services. Working primarily in the field, you will assess needs, develop personalized care plans, and ensure members engage with services to optimize their health outcomes.
Key Responsibilities
Link members with chronic behavioral health and medical conditions to necessary services
Develop and implement person-centered care plans
Provide outreach services and respond to member inquiries
Track and manage appointments and referrals
Collaborate with members and families to assess needs and compliance
Required Qualifications
Bachelor's degree in social work, psychology, nursing, rehabilitation, education, or related field
Two years of experience providing direct services or case management to mentally disabled, chronically ill, or homeless individuals
Preferred Qualifications
Experience working in interdisciplinary teams
Experience in care management or coordination in a medical/behavioral health environment
Fluency in a second language such as Spanish, Russian, or Creole
