Job Description
Skyline Ridge Nursing & Rehabilitation Center seeks a PRN Physical Therapist to improve residents' mobility and independence. This flexible role involves evaluating and treating residents while collaborating with a multidisciplinary team to provide high-quality care.
Key Responsibilities
Perform comprehensive evaluations of residents’ physical abilities and needs
Develop and implement personalized treatment plans
Provide therapeutic interventions, including exercises and manual therapy
Monitor residents’ progress and modify therapy plans as needed
Collaborate with an interdisciplinary team
Educate residents and their families on therapy goals and exercises
Document evaluations, treatment sessions, and progress notes
Participate in quality improvement initiatives
Required Qualifications
Doctorate or Bachelor’s/Master’s degree in Physical Therapy from an accredited program
Current and valid Physical Therapist license in the state of Colorado
Strong communication and teamwork skills with a resident-centered approach
Ability to work independently and adapt to a flexible PRN schedule
Dedication to providing compassionate and effective therapy services
Preferred Qualifications
Experience in skilled nursing or rehabilitation settings
