Key Responsibilities
Monitor the health conditions of individuals served
Administer medications and provide documentation
Provide prescribed medical treatment by physician’s orders
Conduct assessments and implement health care services
Provide training to staff members on various topics
Serve as liaison and advocate with other medical service providers
Provide training for individuals served
Required Qualifications
Associate’s Degree in Nursing or related field
Graduate of an accredited LPN program
One year of experience working in a nursing capacity
Current state LPN or LVN License
Current driver's license, car registration, and auto insurance
Additional certifications such as CPR/FA and HIV/AIDS certification, BBP, and ACT training
Effective communication skills
