Job Description

Join Optum as a Field Clinical Practice Performance Specialist where you'll enhance health outcomes by coordinating care for patients in physician practices. This role involves chart reviews, member assessments, and collaboration with healthcare teams to ensure quality care and support health equity.

Key Responsibilities

  • Provide care coordination through physician practices
  • Conduct telephonic member needs assessments
  • Interact with members to schedule appointments and follow up
  • Review member charts and create alerts for opportunities
  • Partner with administrative and clinical staff for member data management
  • Maintain professional relationships with members and providers
  • Facilitate member referrals to special programs
  • Assist members in accessing community and third-party resources
  • Collaborate with interdisciplinary teams for care coordination
  • Educate members regarding healthcare needs
  • Facilitate member compliance with health plans
  • Identify compliance barriers and formulate action plans
  • Document member interactions in programs
  • Maintain focus on customer service and confidentiality

Required Qualifications

  • High School Diploma/GED (or higher)
  • 2+ years of experience working in a hospital, acute care or healthcare
  • 2+ years of experience working in a customer service/call center role
  • Intermediate level of computer/typing proficiency
  • Ability to travel to provider offices within service area
  • Access to reliable transportation and possess a valid US driver’s license

Preferred Qualifications

  • Associates degree or higher
  • Problem solving skills
  • Experience with HEDIS and EMR
  • Experience with navigating and analyzing reports in Microsoft Excel
  • Medicaid, Medicare, Managed Care experience
  • Prior Physician office experience
  • Experience working with vulnerable populations
  • Call Center experience
  • Excellent verbal and written communication skills
  • Demonstrated ability to identify with a consumer
  • Demonstrated ability to perform effective active listening skills
  • Demonstrated ability to take responsibility and accomplish goals
