Job Description
Primary Vascular Care seeks a Nurse Practitioner to conduct on-site clinical assessments and develop treatment plans for residents in long-term care settings. This role involves working independently in a mobile practice and collaborating with various healthcare professionals to ensure effective patient care.
Key Responsibilities
Perform on-site screening clinical assessments to determine patient treatment plans
Function independently to perform history and physical examinations for complex patients
Order and review diagnostic and therapeutic tests
Assist with the provision of care according to regulations
Monitor the effectiveness of interventions
Maintain relationships with contacts in facilities
Educate family members and clients about services and programs
Assist with the development of physician relations
Complete reports in a timely manner
Required Qualifications
Bachelor's Degree
Degree from an accredited Nurse Practitioner or Physician Assistant Program
Current NP License
BLS - Basic Life Support certification
Current Infection Control Certification
Board Certified/Board Eligible
Physical, Vaccines and Titers: PPD/TB, Hep B, MMR, Varicella
Preferred Qualifications
ACLS - Advanced Cardiac Life Support certification
