Key Responsibilities
Conduct comprehensive bio-psychosocial assessments and establish care plans
Collaborate with interdisciplinary teams for care coordination
Provide direct patient care including case management and crisis intervention
Educate patients and families on health maintenance and treatment adherence
Maintain patient-centered documentation
Participate in team meetings and provide supervision to social work students
Support development of hospital programming
Required Qualifications
Master's Degree in Social Work, Counseling, Marriage and Family Therapy or related field
LCSW, LPC, LMFT certification
Previous experience in a healthcare setting
Valid Driver's License
Computer proficiency
Excellent communication skills
