The Registered Nurse Case Manager is responsible for coordinating patient care across inpatient, home health, and skilled nursing settings. Responsibilities include:

• Conducting case management and utilization review using InterQual and Milliman guidelines.
• Coordinating care plans, discharge planning, and regulatory compliance with CMS and Department of Health standards.
• Applying knowledge of Medicare, HMO, CPT/ICD-10 coding, DRG classifications, and insurance requirements.
• Ensuring adherence to HIPAA, OSHA, The Joint Commission, and NCQA standards.
• Managing a daily caseload of 15 patients, with weekend ratios at 1:40.
• Collaborating with interdisciplinary teams including social services and transportation support.
• Completing accurate documentation in Epic and ensuring timely discharge and transition of care.Ax
