Key Responsibilities
Link adults and children with chronic behavioral health and medical conditions to necessary services
Develop and implement personalized care plans
Conduct periodic assessments of members' health needs
Provide care management services and track referrals
Respond to member crises and information inquiries
Ensure ongoing access to services as defined in care plans
Required Qualifications
Bachelor's degree in social work, psychology, nursing, rehabilitation, education, or related fields
Two years of experience providing direct services or case management to mentally disabled or chronically ill individuals
Experience working in interdisciplinary teams
Experience providing care management or care coordination
Preferred Qualifications
Fluency in a second language such as Spanish, Russian, or Creole
