Job Description
Join HarmonyCares as a Nurse Practitioner (PRN) and deliver personalized, quality healthcare in home settings. You will conduct health assessments, collaborate with healthcare teams, and make a meaningful impact on patient care while enjoying a flexible schedule and supportive work environment.
Key Responsibilities
Conduct comprehensive in-home health risk assessments
Take patient history and examine to determine diagnoses
Provide written documentation of patient visits per NCQA standards
Take patient vital signs as necessary
Place case management referrals and communicate with PCP as necessary
Maintain patient confidentiality
Participate in professional development activities
Required Qualifications
Active/unrestricted medical license
Active CPR Certification
Board eligible or board certified in family medicine
Outstanding EHR skills
Preferred Qualifications
Geriatric training/experience
Skill in teamwork and maintaining effective working relationships with patients, medical staff, and the public
