Key Responsibilities
Evaluate member needs and barriers to care
Develop ongoing care/service plans
Identify problems/barriers to care and provide interventions
Coordinate between member and care provider team
Monitor member status and progress toward care plan goals
Provide resource support for local services
Facilitate care management and ensure timely access to needed care
Collect and document member information and care management activities
Educate members and families on healthcare options and benefits
Provide feedback on care improvement opportunities
Required Qualifications
Degree from an accredited school of nursing or Bachelor's degree in Nursing
2-4 years of related experience
RN - Registered Nurse - State Licensure and/or Compact State Licensure
Preferred Qualifications
Bilingual Spanish speakers
Two years of pediatric experience
