Key Responsibilities
Oversee daily operations, staffing, and workflows for the Care Management Department
Manage a team of nurses and social workers
Provide care management services including care coordination, discharge planning, and social services
Collaborate with medical and nursing staff to manage patient care and length of stay
Lead continuous process improvement activities
Track and report on department metrics
Required Qualifications
Bachelor's degree in a related field
BSN required; MSN preferred
5-7 years related experience including supervisory experience
Prior experience with inpatient case management/discharge planning
Maryland Registered Nurse License or intent to achieve MD licensure
Preferred Qualifications
Case Management certification within 3-5 years of hire
