Job Description

Key Responsibilities

  • Develop and implement care plan interventions
  • Communicate required health-related information to stakeholders
  • Advocate for patients and families
  • Assess, plan, and implement individualized care strategies
  • Identify problems/barriers to care and provide management interventions
  • Initiate referrals for social service programs
  • Provide resource support to members for local services
  • Manage the person-centered service/support plan
  • Conduct home visits and in-person visits
  • Document all member information and care management activities

Required Qualifications

  • Current, unrestricted independent licensure as a Registered Nurse (RN) in Michigan or a Master’s/Bachelor’s degree in Social Work with appropriate licensure
  • 2+ years of experience in community health setting
  • 1+ years of experience with local behavioral health providers and community support organizations
  • 1+ years of experience with long-term care needs and/or home and community-based services
  • 1+ years of experience with electronic documentation systems and MS Office
  • Access to reliable transportation and ability to travel within assigned territory
  • Access to a designated quiet workspace at home
  • Ability to travel to Southfield, MI for quarterly meetings
  • Must reside within Michigan

Preferred Qualifications

  • RN or LMSW; LCSW, LLMSW
  • 1+ years of medical case management experience
  • Experience in Motivational Interviewing, Stages of Change, Trauma-Informed Care
  • Experience with individuals with co-occurring disorders
  • Experience with MI Health Link (MMP)
  • Experience in Managed Care
  • Knowledge of NCQA documentation standards
