Job Description
Join The Jewish Board as a Case Associate to make a meaningful difference in the lives of clients. You'll develop action plans and connect clients with essential resources while working closely within their communities. This role offers the opportunity to impact the well-being of individuals and families in New York City.
Key Responsibilities
Develop action plans for client care
Perform routine community contacts and home visits
Evaluate living conditions and client needs
Provide case management and individual/group counseling
Facilitate client referrals to treatment or service providers
Conduct community outreach
Maintain necessary documentation
Required Qualifications
BA Degree
Experience in a community or social service group or five years as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
