Key Responsibilities
Accountable for direct and indirect quality care for patients of all ages
Coordinate patient care activities with other members of the health care team
Follow the nursing process and adhere to the Nurse Practice Act
Provide care in patients' homes, fostering rewarding relationships
Required Qualifications
Education to meet certification, license or registration requirement
1 year medical/surgical experience in an acute care setting or mental health experience
National Provider BLS - American Heart Association within 30 days of hire
Oregon Registered Nurse License upon hire
Washington Registered Nurse License upon hire (if practicing in this state)
Preferred Qualifications
Recent Home Health experience
Community mental health experience
Geriatric experience
ANCC Psychiatric for Mental Health RN work assignment
Hospice or Palliative Care experience
Point of Care experience
