Key Responsibilities
Conduct thorough patient and family assessments for care and discharge planning
Coordinate patient care and communicate with multidisciplinary healthcare team
Ensure patient understanding of diagnosis and treatment plan
Develop and implement discharge plans
Advocate for patient care issues and analyze care trends
Participate in quality improvement processes
Required Qualifications
Current RN license in Connecticut
Minimum of 3 years acute care hospital experience
Strong communication and interpersonal skills
Knowledge of the nursing process
Preferred Qualifications
Bachelor’s degree in Nursing
Care Management experience
Case Management certification (ACM or CCM)
