Position Overview
We are seeking a skilled and motivated Physical Therapist to join a leading home health team serving Napa and nearby counties. This role focuses on providing personalized in-home physical therapy services to help patients regain mobility, function, and independence in the comfort of their homes.
Key Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans.
- Deliver home-based physical therapy treatments focused on recovery and mobility improvement.
- Document care plans, progress notes, and updates in accordance with agency standards.
- Communicate effectively with patients, families, and interdisciplinary healthcare teams.
- Travel within assigned territories and provide weekend coverage as needed.
- Maintain compliance with all safety, documentation, and regulatory standards.
Qualifications
Education & Licensure:
- Graduate of an accredited Physical Therapy program.
- Active California PT license — Required.
- BLS certification — Required.
- Valid Driver’s License , Auto Insurance , and reliable transportation.
Experience:
- Minimum of 1 year of Physical Therapy experience ; home health experience preferred.
- Must be willing to work with COVID+ or COVID PUI patients if needed.
Additional Requirements:
- Compliance with all health clearances (TB, MMR, Varicella, Hep B, Tdap, Flu, Drug Screen, Physical).
- Must be flexible with travel and weekend assignments as needed.
Workload & Expectations
- Assigned to multiple counties within designated service areas.
- Productivity target: 27–30 points per week (each visit type corresponds to defined point values).
- 36-hour weekly guarantee applies only when productivity and travel requirements are met.
- Payment structure based on per-visit productivity model ; rate includes drive time, documentation, and communication.
