Job Description
Join The Jewish Board as a Case Associate and make a significant impact in the lives of New Yorkers. You will be responsible for developing care plans, connecting clients to resources, and providing counseling to support their needs.
Key Responsibilities
Develop action plans for client care
Routine community contacts on behalf of social worker
Locate and utilize community resources for clients
Evaluate living conditions through home visits
Provide individual/group counseling
Perform community outreach and establish linkages with other organizations
Provide case management to clients
Document necessary notes and progress reports
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual Spanish
Bilingual Haitian-Creole
