Key Responsibilities
Evaluate and treat center patients according to standards of care
Monitor care using available data and chart reviews
Assist in coordinating patient services including specialty referrals
Participate in daily huddles with the care team
Complete medical record documentation in a timely manner
Participate in quality improvement initiatives
Engage in local primary care on-call program as needed
Ensure compliance with licensing and accrediting bodies
Required Qualifications
Current and unrestricted medical license
Graduate of accredited MD or DO program
Excellent verbal and written communication skills
Demonstrated skill in interpersonal relationships
Engagement in integrated team-based care
Willingness to adapt to a value-based care setting
Superior patient/customer service
Basic computer skills including email and EMR
Preferred Qualifications
Board Certification or eligible in Family Medicine, Internal Medicine or Geriatric Medicine
Active and unrestricted DEA license
Medicare Provider Number
Medicaid Provider Number
2-5 years applicable experience
Experience managing Medicare Advantage panel of patients
Knowledge of Medicare guidelines and coverage
Bilingual
Knowledge of HEDIS quality indicators
