The Care Coordinator links individuals with chronic behavioral health and medical conditions to necessary services, working primarily in the field. This role involves assessing needs, developing care plans, providing management services, and ensuring access to a variety of health and community services.
Key Responsibilities
Link members to health services and optimize health outcomes
Assess risk and needs, develop care plans, and provide care management
Track and arrange appointments and educate members
Respond to member crises and provide outreach services
Complete documentation and share knowledge with team members
Required Qualifications
Bachelor's degree in social work, psychology, nursing, rehabilitation, education, or related fields
Two years of experience providing direct services to mentally disabled or chronically ill individuals
Experience working in interdisciplinary teams
Experience providing care management or care coordination in a medical or behavioral health environment
Preferred Qualifications
Fluency in a second language such as Spanish, Russian, or Creole
Experience working with children or the chronically ill
