Job Description

Join Monte Nido & Affiliates as a Hybrid Social Worker, where you will provide essential care coordination and case management for clients and families across multiple programs. Your role will be crucial in ensuring smooth transitions between care levels and connecting individuals with community resources, all while making a positive impact on their recovery journey.

Key Responsibilities

  • Care Coordination & Case Management: Partner with the clinical team to offer case management and care coordination for clients and families.
  • Community & Family Support: Assist in connecting clients and families with community-based programs and resources.
  • Referral Management: Send referrals to community programs and facilitate collaboration with outpatient providers.
  • Transitions Between Care Levels: Help facilitate smooth transitions between various levels of care.
  • Prepare Clients & Families for Transition: Provide necessary information and emotional support.
  • Ongoing Resource Connection: Ensure clients continue to receive necessary care after transitioning.

Required Qualifications

  • Master’s degree in Social Work or related field
  • Experience with CareOregon/Medicaid
  • Proven experience in case management or care coordination, preferably in behavioral health or residential treatment settings
  • Strong communication skills and ability to work collaboratively with a multidisciplinary team
  • Ability to build relationships with clients, families, and community providers
  • Understanding of available community resources, government programs, and insurance processes

Preferred Qualifications

  • LCSW preferred
